I am so confused about this?

I'm a boy but ever since I was like 12 I've been wearing womens clothes heels especially and no I'm 16 gonna be 17 soon and I am really confused I stil like girls but I do feel like I should be a girl I have even decided to go by the name Jeanette I want to get a surgery and I want to live life as a female but I still have this feelins of being embarresed and scared about it could anyone help me
